It is considered as the important roles of pharmacists to review prescriptions before starting dispensing. We have analyzed the details of telephone or written inquiries on the prescriptions collected from January, 1986 to December, 1986 for one year. Frequency of inquiries was about 1.1£¥ or 410 prescriptions per month, (4925 out of 453,358 prescriptions from inpatients and outpatients). The details of the inquiries were dosage (28.6£¥), drug name(27.1£¥), strength(13.2£¥), insufficient writting of presciption to the rules(8.3£¥), administration schedule, dosage form, period of medication and so on. As to the writing errors of drug names which are most frequently occur, the causes were also analyzed. As a result of the review we can be concluded that it is desirable to educate doctors about prescription writing to decrease prescription errors.
